Facility Reservation Forms

I. To Reserve A Community Center or A Reservable Picnic Site:

Please email your completed application and rules form along with your driver’s license or state ID to email at prm-info@longbeach.gov .

Submission of the completed application form DOES NOT confirm a guaranteed reservation. Bookings are on a first-come, first-served basis. Payment in full for reservation will be due within 10 calendar days after the reservation is confirmed. For more information, please call the Registration/Reservations office at (562) 570-3111.

II. To Get A Permit for Open Space (Permit to Gather) for Groups of 25 or more or for a Bounce House Permit

2. Download the Moon Bounce authorized list from the homepage. Select and make arrangements with an authorized moon bounce vendor from the list. Please indicate on the application form the moon bounce company selected from the authorized list of Moon Bounce Vendors, how many and type of moon bounce. 

(No personal moon bounces or soft plays allowed)

A picnic Permit to Gather is required for groups of 25 persons or more under LB Municipal Code, Section 16.16.020. It is a permit to gather ONLY and does not entitle, reserve, or guarantee any specific area or any amenities and does NOT include the use of any reservable sites or athletic fields. Please contact the Sports Office (562) 570-3204 for athletic field permits. For more information, please call the Registration/Reservations Office at (562) 570-3111.

III. Department Fee Waiver Policy and Forms:

1. Determine your group's eligibility for a possible reduction of rental fees by reviewing the Fee Waiver Policy 
2. Fill out the Fee Waiver Form
3. Fill out the Levine Act Disclosure Statement, if the applicant or organization answered yes to City Officer contributions
4. Fill out the  Application and Rules for Facility, Senior Center, Picnic Areas, and Open Space
5. Submit #2, #3 (if applicable), & #4 along with a copy of your driver’s license to prm-info@longbeach.gov. Please submit fee waiver requests a minimum of 60 days prior to the event to allow time for processing.
